Evergreen Horizon Care & Support provides high-quality, safe, effective and compassionate care for adults living in the community. We help people maintain independence and achieve better wellbeing outcomes through individualised assessments, risk management, and co-produced care plans that reflect each person’s preferences, goals, culture and daily routine.

Evergreen Horizon Care & Support provides regulated personal care to adults in the community. We support people to live independently in their own homes and, where commissioned, within supported living settings — with strong safeguarding, professional governance and person-centred planning embedded in everyday practice.

Service Model

Individualised support plans co-produced with people, families, advocates and commissioning professionals — reviewed regularly:

Domiciliary Care — Regulated personal care delivered primarily through domiciliary care, supporting adults in their own homes with dignity, choice and independence.
Supported Living — Where commissioned, personal care within supported living settings in partnership with housing providers and Local Authorities — individuals retain tenancy/ownership and receive flexible, non-institutional support.
Live-in Care — Live-in support designed to provide continuity and stability, with routines and goals agreed and reviewed in line with assessed needs and outcomes.

Staffing and Management — Evergreen Horizon Care & Support employs trained, competent, and compassionate staff supported through regular supervision, training and professional development. The service is led by a Registered Manager who ensures compliance with CQC regulations and promotes a positive, inclusive workplace culture.
